Direct feedback from our patients/families and client facilities is extremely valuable to us. By reviewing your experience with us, we can address concerns, identify areas for improvement, as well as celebrate the things we do well.

Patient Satisfaction Survey and Facility Satisfaction Survey are both available here online as well as on paper (if you request a paper format from our office). Also, if you need help completing the survey, please let our office staff know and we will provide you with the necessary assistance.

Both Patient and Facility Satisfaction Surveys can be completed confidentially without disclosure of names. Of course, there is an option for providing a name if the individual chooses to disclose that. Our practice management staff review submissions and respond accordingly.
